How to use this COVID-19 employee vaccination policy and example

The Occupational Safety and Health Administration (OSHA) has finalized its emergency temporary standard (ETS), which strongly encourages vaccination to protect unvaccinated employees of large employers (100 or more employees) from contracting COVID-19.

Covered employers may soon be required to create, apply, and enforce a mandatory COVID-19 vaccination policy for all employees, or adopt a policy that requires employees to either get the vaccine or wear a face covering at work and submit to weekly COVID-19 testing.

This policy is customizable, so you can adopt it for either (1) mandatory vaccination or (2) vaccination and masking/testing.

What you'll find in this vaccination policy template

  • Purpose: Why this policy has been put in place, to provide guidelines pertaining to the expectations and requirements of staff with respect to COVID-19 and vaccination.
  • Scope: This policy includes employees across all locations or worksites, regardless of vaccination status, part-time or full-time status, or where you perform your work (including remote employees). As per the ETS, employees who never interact with other individuals indoors are exempt. This includes remote workers, as well as those who work exclusively outdoors.
  • Definitions: Fully vaccinated, partially vaccinated, acceptable proof of vaccination.
  • Details of the vaccination policy: If it’s mandatory, the deadline, how to file for an exception, etc.
  • PTO policy: Following the guidelines laid out by the ETS.
  • Confidentiality and privacy: What the vaccination information will be used for and how privacy will be maintained in the event of a positive COVID-19 test.
  • How unvaccinated employees can (or if) they can continue to work
  • What the company will do in the event of a positive COVID-19 test result
