How to use this employee engagement survey template
This template is designed to provide you with an overview of employee engagement within your organization. Here’s how to make the most of it:
- Distribute regularly: Whether bi-annually or quarterly, determine a survey cadence that allows you to regularly monitor engagement levels and identify trends over time.
- Allow anonymity: Encourage honest and open feedback by giving your employees the option to complete the survey anonymously. Of course, if they want to be contacted for further conversation or follow up, there’s a field for that, too!
- Analyze and share the results: Review the collected data to pinpoint strengths and areas of opportunity, then share what you’ve found with the company! Consider doing this at an All Hands meeting or your next retreat.
Take action: Develop and implement strategies based on the feedback you got from employees to enhance their overall satisfaction and performance over time.

What you’ll find in this employee engagement survey template (+ quick tips!)
This employee engagement survey template is modeled after one used by Workshop’s own Internal Communications Manager, Devin! It’s designed to take less than ten minutes to complete and is broken down into six key sections. To keep things simple for respondents, each section is either a scaled agreement (e.g., ‘strongly agree’–’strongly disagree’) or free-response questions.
Whether you use this template completely or decide to tweak it a bit, here are a few tips to maximize your success:
- Keep it concise: Aim for a survey that takes no more than 15 minutes of employee time to encourage higher response rates. Don’t forget to send reminders to your employees to complete the survey!
- Use a mix of question types: Incorporate multiple-choice, likert scale, and open-ended questions to gather both quantitative and qualitative data.
Pilot the survey: Test the survey with a small group before full deployment to identify any ambiguities or technical issues.
